COVID-19 County Relief Block Grant Application for Non-Profits

Who is Eligible to apply?

  • Organizations that are and/or were negatively impacted by COVID-19, including loss of revenue.
  • Organizations must be physically located in Centre County.
  • Organizations must deliver services in Centre County.
  • Organizations must be a 501(c)3 or 501(c)19 (Per Act 24) in Centre County.

At present, this is expected to be the only non-profit application round. Applications are due Monday, September 28th at 5:00 pm. 

 

This is a non-profit grant and does not require any pay back.  There is no matching-funds requirement for this grant.

 

You are eligible to apply if you received other CARES Act funding such as the Paycheck Protection Program (PPP) or Economic Injury Disaster Loan Program (EIDL), but you must identify that you received the funds. 

 

Centre County is partnering with Centre Foundation on the application process. Centre Foundation will receive and vet all applications to assure the applications are complete and applicants are fully qualified. Once that processing and vetting is completed, all application information will be sent to Centre County for funding decisions.  Centre Foundation will not have any role in the grant decision process.


To complete the application you will need the following items.  It will be helpful to prepare these prior to starting.

  1. General non-profit information (EIN, type of non-profit entity, other non-profit related information.)
  2. Income statement for March 1, 2019 through July 31, 2019 and also from March 1, 2020 through July 31, 2020.
  3. If you expect future loss from August 1, 2020 through December 30, 2020. 
  4. Your latest 990 tax return.  
  5. Form W-9 (Link)
  6. IRS Letter of Determination for your organization
  7. Explanation of the impact of COVID-19 on your non-profit.
