Homemaker Services
Homemaker Services are offered by County Homemakers and Home Helpers via referral from the Office of Adult Services via Housing Transitions' Service Planning Case Management program. This program provides in-home services to eligible residents (ages 18-59), with a disability, who cannot perform routine chore assistance and/or require assistance with non-medical personal care. Homemaker services offer supplemental assistance when natural supports (family and friends) are not an option. Other Resources - "Under 60" Waiver Programs

Attendant Care/ACT 150 Waiver
If you have a physical disability, the Attendant Care Waiver and state funded Act 150 program may be available to you to continue to live in your home and community with support services.

Independence Waiver
If you are an adult with a severe physical disability, the Independence Waiver may be able to help you live or remain in the community and remain as independent as possible.
